School Age Request for Support

Please ensure you have the evidence of an identified and persistent speech language and communication need, as you will need to refer to this and submit it with the request for support form.

Information on types of evidence to submit

Completing a Request for Support

You will also need the following information in order to complete the request form:

  • Child’s full name, date of birth and home address
  • Child’s GP name and address
  • Parent/carer names
  • Parental consent (verbal with date and who consent was provided to)
  • Safeguarding information, where relevant
  • Names and contact details of any other professionals involved
  • Information of any medical diagnoses
  • Evidence of your initial speech/language screen, intervention record, and rescreen (speech sound screen if appropriate).

Next Steps

Mainstream primary schools and complex needs schools under local authority control in Norfolk and Waveney are offered a link planning meeting with their link Speech and Language Therapist each term. These meetings take place via video call.

Link planning meetings provide an opportunity for the school SENCo (and additional relevant staff e.g. speech and language teaching assistant) to discuss with a therapist:

  • Children currently known to SaLT
  • New children where a Request for Support has been received
  • Speech and language support already in place at the school (e.g. screening and intervention tools used)
  • Signposting to training/resources

For other settings that do not have link planning meetings e.g. secondary schools, independent schools and some specialist settings, a therapist will be in touch to arrange an appointment in due course following a request for support being received.

Once your request for support and supporting information is completed please email to:
